Well...

I would have to say Photobucket (http://photobucket.com/). It's free, you can get an account that you can re-use images in AND you can use it to set a custom avatar in the chat program. Your images are saved forever.

Alternatively you can use Image Shack (http://imageshack.us/), which has sometimes some REALLY nasty ads but doesn't make you sign up before using it. I think your images are deleted after a while.

I highly recommend you use Photobucket.

Just MAKE EVERYTHING UP! Go to google maps and type in some cities or states and pick one out if you really need to. Make up a fake name and everything. It doesn't matter. It *REALLY* doesn't.

On-topic: I too recommend Photobucket. For a long time I've been using that site in addition to my own, and they have been reliable so far.
